Pain and inflammation can be related, but they are not the same thing. Pain does not always involve inflammation, and a finding about an inflammatory marker does not establish pain relief. This guide explains the distinction, introduces different pain mechanisms and points to research without treating every condition as one problem.
Different pain mechanisms
The International Association for the Study of Pain distinguishes several terms. Nociceptive pain relates to actual or threatened damage to non-neural tissue. Neuropathic pain is caused by a lesion or disease of the somatosensory nervous system. Nociplastic pain involves altered nociception without clear evidence of the tissue damage or nervous-system lesion that would explain it through those other mechanisms. Read the IASP terminology.
These are explanations of mechanisms, not a checklist for diagnosing yourself. More than one mechanism can contribute. Pain is a personal experience influenced by biological, psychological and social factors; the absence of an obvious injury does not make it unreal.
What inflammation means
Inflammation is normally part of the body's response to infection or damaged tissue. It helps coordinate defence and healing. Chronic inflammation describes an inflammatory response that continues when it should resolve or starts without the appropriate trigger. The National Cancer Institute explains inflammation.
That is why “anti-inflammatory” is not a universal answer to pain. A change in a laboratory measurement and a person's ability to move, sleep or function are different outcomes. Research needs to establish which changed, in whom and by how much.
Managing persistent pain
The NHS describes several approaches that can form part of managing persistent pain, including suitable activity, breathing exercises, talking therapies and physical therapy. The useful combination depends on the person and the cause; a clinician can help you choose appropriate options. Read the NHS pain-management guidance.
Bring a practical description to an appointment: where the pain occurs, when it started, what changes it and how it affects everyday activities. Seek advice for unexplained, severe or worsening pain rather than trying to identify it from an ingredient article.
Botanicals and CBD: keep the question specific
A study of one preparation in one pain condition cannot establish a treatment for all pain or inflammation. Distinguish human trials from laboratory work, short-term symptom scores from longer-term outcomes, and the tested formulation from retail products.
